Mission Statement

AOS Surgery Center, LLC, continually strives to provide high-quality outpatient orthopedic surgery that is convenient, patient-centered and cost-effective for the CSRA, as well as surrounding communities. We are committed to providing excellent, effective care, while treating everyone we serve with compassion and kindness.

To start the preop assessment process, AOS Surgery Center requests you complete your medical history by clicking the Online Clinical History button below to create your One Medical Passport.

Please complete and submit your Online Clinical History as soon as your surgery has been scheduled. This will allow our preop nurse to review your history and prepare for your surgery. You will receive a phone call from a nurse a few days prior to your scheduled procedure regarding your arrival time, as well as, any additional instructions.

You should have the following information available before beginning your Online Clinical History:

• Health insurance information
• Names and phone numbers of your physicians
• A list of all medications you are currently taking including dosages and frequencies
• List of surgical procedures you have had in the past
• Preferred pharmacy and phone number

About Your Preoperative Assessment

SIS Complete is a secure website that allows you to enter your information at any time from anywhere. 

Once your surgery is scheduled, you will receive an email or text with a message containing a link to access SIS Complete.  Please be assured your information is kept confidential and will be thoroughly reviewed by your medical team.
